Many forms require signatures from a Notary Public or a DMV representative to be valid. These may be filled out in advance, but you must wait to sign and date them until you are actually at the Notary or DMV Office.

Authorizes the DMV to charge your credit or debit card for license or registration fees.

• Minor Affidavit (DP 38)
  Certifies that a minor is aware of various requirements and laws. Also used by a parent/guardian to cancel a minor driver license or instruction permit.
• Certification of Attendance (DMV 301)
  For drivers under 18 to show proof of school attendance.
• Beginning Driver Experience Log (DLD 130) | Additional Time Sheets
  For certification of Behind-the-Wheel Driving Experience for drivers under 18.
• Driver Certification of Eligibility for Non-Commercial Driver’s License - Farm/Military (DLD 121)
  To claim exemption from federal commercial licensing regulations.
• Driver Certification of Eligibility for Non-Commercial Driver’s License - Firefighters (DP 046)
  To claim exemption from federal commercial licensing regulations.
• Declaration of Homeless Status (DMV 128)
  For the homeless to apply for a one-time waiver for a duplicate license or ID card.
• Request for Waiver of Duplicate Fees - Released Prisoners (DP 190)
  For recently-released prisoners to apply for a one-time waiver for a duplicate license or ID card.
• Released Prisoner or Inmate Driver’s License and Identification Card Information (DMV 013)
  Proof of identity and other requirements for prisoners recently released from a Nevada institution.
• Application for Alternate Address (DMV 007)
  For applicants authorized by law to list an alternate address on their license. These circumstances are very rare - if you're one of these people, you already know it.
• Drive Test Waiver for Taiwan Nationals (DMV 015)
  For licensed drivers from Taiwan moving to Nevada. Taiwan has a reciprocity agreement with the State of Nevada to exchange a Taiwanese driver licenses for a Nevada license.

Restricted Licenses, Suspensions and Revocations

  To apply for a standard restricted license during a license revocation, for medical hardship, school transportation or child visitation.

  To apply for a restricted license for those ordered to participate in the 24/7 Sobriety and Drug Monitoring Program.

Residency

  To enable another person to provide proof of an applicant's residency.
• Relief Agency or Shelter Certification (DMV 115)
  To enable social service agencies to provide proof of the applicant's residency.

• Change of Address Notification by Mail (DMV 22)
  Applications are also available in Spanish and Tagalog

• Application for Duplicate Registration Certificate (VP 013)
  Try our Online Duplicate Registration . There is a $5 fee for each certificate requested. You may mail this application and a check, money order or a Payment Authorization to the address on the form. Or, you may fax the form and Payment Authorization to (775) 684-4797. • Change of Address Application (MC 32)
  Needed to renew or change a license or registration. Must be signed by the owner.
• Application for Vehicle Registration (VP 222)
  Needed when applying for an original registration. Must be signed by the owner.
• Affidavit of Non-Operation (VP 018)
  Certifies that a specific vehicle has not been operated on Nevada public highways.
• Vehicle Inspection Certificate (VP 015)
  For Vehicle Identification Number Verification. Must be completed by a DMV representative, a peace officer or other authorized agent.
• Moped Registration Application/Inspection (VP 30)
  Do not mail this form. The inspection must be completed by a DMV representative, Sheriff or Deputy Sheriff.
• Emission Control Exemption Application (EC 008)
  For Nevada residents temporarily out of state who are unable to obtain a smog check.
• Odometer Certification for Emission Exemption (EC 018)
  To claim the emission inspection Exemption for vehicles registered as a Classic Rod or Classic Vehicle.
• Affidavit of Non-Residency (VP 198)
  To register vehicles which were purchased in Nevada, taken out-of-state and then returned to Nevada.
• License Plate Hearing Request (SP 75)
  To request an Administrative Hearing for a denied or recalled personalized license plate number.
• Registration Service Transmittal Form (VP 193) | Registration Service Transaction Request (VP 247)
  Note: All document preparation services are required to be registered with the Nevada Secretary of State .
• Application for Non-Resident Business Permit (VP 264)
  For commercial vehicles under 10,001 pounds. See Movement Permits - Commercial for details.
• Military Surplus Vehicle Inspection (VP 017)
  For vehicles labeled for "OFF-ROAD USE ONLY" - cannot be converted for on-road use with the exception of an off highway two-wheeled motorcycle.

  For Nevada Native American Tribal Members residing on tribal lands.
• Farmer/Rancher Affidavit (VP 159)
  Used to adjust registration fees on large farm vehicles.

  You must obtain new plates in person at a DMV Full Service Office .
• Duplicate License Plate Application (SP 14)
  Plates to replace damaged or unreadable plates in the same style. Not for stolen plates.

Insurance

• Vehicle Registration Insurance Suspension Guide (NVL 010)
  Items to check if the DMV is reporting a lapse in your insurance coverage.
• Declaration of Responsibility (NVL 019)
  To declare that you will meet financial responsibility requirements. See Insurance .
• Dormant Vehicle Affidavit (NVL 003)
  Used to reinstate a vehicle registration which has been suspended for failure to maintain liability insurance. Only used if the vehicle has not been operated during the lapse of insurance.
• Property Owner Affidavit (NVL 025)
  To declare that a vehicle was stored at a specific address.

  This form is used if there is no Manufacturers Certificate/Statement of Origin or outstanding Certificate of Title.
• Bill Of Sale (VP 104)
  Releases a private party's ownership interest in a vehicle.
• Application for Duplicate Nevada Certificate of Title (VP 012)
  This form is for use only if the vehicle was last titled in Nevada. If you are not sure whether your vehicle is titled in Nevada or if you do not have all of the information requested, please contact the Records Section for instructions. If your vehicle is titled in another state, you must obtain a duplicate from that state.
• Lien Release (VP 186)
  Releases an ownership interest held by a financial institution or other third party.
• Electronic Lienholder Reassignment Affidavit (VP 268)
  Assigns lienhollder rights on electronic titles.
• Attempt to Contact (VP 212)
  Instructions for attempting to contact the owner of a vehicle.
• Lost Title Affidavit (VP 206)
  Certifies that a recently-issued title was not received.
• Power of Attorney (VP 136)
  Used to convey the authority to transfer the ownership of or register a vehicle.
• Affidavit of Acknowledgment for a Power of Attorney (VP 249)
  To certify a true copy of a Power of Attorney.
• Erasure Affidavit (VP 019)
  Used to state reasons for changes and errors in completing the buyer or reassignment sections of vehicle titles.

  Used to clarify the identity of a vehicle owner.

Estates and Trusts

• Affidavit for transfer of Title for Estates Without Probate (VP 024)
  For a deceased person's heirs, estate or creditors to claim ownership of a vehicle.
• Trustee Appointment and Powers Affidavit (VP 188)
  To declare a person has trustee powers to transfer ownership of a vehicle.
• Transfer on Death Application (VP 239)
  To add, change or delete a beneficiary designated to receive a vehicle upon death the owners.

DMV Licensees & ID Theft

The DMV Compliance Enforcement Division (CED) accepts complaints against auto dealers, lessors, emissions stations, driving schools and other businesses which are regulated by the DMV.

CED also investigates identity theft and other cases involving fraud, misuse or theft regarding DMV processes or related documents.
